Peter Paul’s After School Program Application Information
Please fill out one form for each student applying to the 2019-2020 After School Program. We serve students in grades 2 – 12. If you have any questions regarding the application or the program, please contact Dr. Stephanie Bassett, Director of Education. The following items will be required for enrollment in Peter Paul’s After School Program:
- Student’s birth certificate
- Documentation of the student’s physical exam
- Student’s immunization record
A copy of a completed School Entrance Health Form is acceptable to take the place of the second two documents. The birth certificate is still required. You may access the School Entrance Health Form online by clicking here.
